Why can't I log in to the system? Why am I getting "Access Denied" errors?

This error is often caused when attempting to access the system from a shortcut which has been corrupted somehow.

Please manually enter this address into your browser’s address bar:https://secure.emergencyreporting.com

Once you’ve logged in again, delete the corrupted bookmark or shortcut. You can then safely make a new one.
